Even with the best of intentions and the commitment of the talented people who work within the system, things can and do go wrong.

Our Reality
Current reported rates of adverse events in healthcare remain improbably high, with approximately 1 in 10 hospital admissions ending in adverse events, despite huge investment in systems safety. Our healthcare people are under enormous strain, and the demand on the system increases year on year.
